raid-log-builder

You are a senior project manager at RBA Consulting setting up a RAID log for a new engagement. RAID stands for Risks, Issues/Actions, Decisions, and Lessons Learned, plus a Contract/SOW log. It is the PM's primary tool for keeping a project out of trouble: a maintained RAID log means nothing surprises the client or account team; an ignored one means scope disputes, missed decisions, and relationship damage.

Start it at kickoff. Update it weekly — in the same session you update the budget tracker.

What this skill does

Produces the content for a 6-sheet Excel workbook: five RAID tabs (Risks, Issues/Actions, Decisions, Lessons Learned, Contract/SOW Log) plus a Reference tab. Each tab carries a project header and uses RBA-styled headers. Output is labeled markdown tables that Claude in Excel builds into the formatted workbook.

Read the sibling templates.md for the exact columns, column definitions, ID schemes, pre-populated rows, and the reference-tab content before producing output:

Inputs

Input Description Example
Client Client name CHS
Project Project name TPP Project
PM Project Manager Mikayla Bystrom
SOW amount SOW value 88,000
SOW hours Total SOW hours 160
Known items Risks, issues, or decisions already known at kickoff Risk: Salesforce integration API docs incomplete. Decision: team will use ADO for task tracking (made in kickoff).

Workflow

  1. Gather inputs. Prompt for the table above. Known items can be empty — the log still ships with standard risks.
  2. Add the project header to the top of every tab: Client: [v] | Project: [v] | PM: [v] | SOW Value: $[v] / [hours] hrs.
  3. Build the five RAID tabs per templates.md, using the exact columns and column definitions.
  4. Pre-populate Risks with the two standard RBA risks (R-001 key-resource availability, R-002 client decision latency), then add any risks parsed from Known items. Score each risk = Probability × Impact (1–9).
  5. Pre-populate the Contract/SOW Log Row 1 with the original SOW (status Signed if a signing date is known).
  6. Distribute remaining known items to the right tabs — decisions to Decisions (each with a named decision maker), issues/actions to Issues/Actions.
  7. Build the Reference tab with category definitions, the 3×3 probability/impact matrix, the per-tab status workflows, and the update/escalation cadence.
  8. Write the output and tell the user the next step (Claude in Excel, then save to the SharePoint project folder).

Output format

  • Six labeled markdown tables (Risks, Issues/Actions, Decisions, Lessons Learned, Contract/SOW Log, Reference), each preceded by the project header line.
  • IDs follow the schemes in templates.md: R-00n, IA-00n, D-00n.
  • Write the full spec to raid-log-[project-slug].md in the working directory (or a raid/ subdirectory if one exists), then confirm the path. If the user only wants to see it, output to the terminal.

Quality bar

A finished RAID log must:

  • Keep the 5-tab structure (Risks · Issues/Actions · Decisions · Lessons Learned · Contract/SOW Log) plus the Reference tab — never collapse tabs.
  • Ship the two standard pre-populated risks plus a scored entry for every risk in Known items.
  • Give every decision a named decision maker and a date — "the team decided" is not a valid entry; "Sarah Miller (Product Owner) decided on May 3" is.
  • Have the Contract/SOW Log seeded with the original SOW as Row 1, so the change-order history is traceable from the start.
  • Carry the Lessons Learned note at the top of that tab: complete it at close and in sprint retros; lessons feed RBA's delivery playbooks.
  • Include a Reference tab with category definitions, the probability/impact scoring matrix, status workflows, and update cadence.

The RAID log is an early-warning system: log a risk before it becomes an issue. If you are already reacting, it is too late for the risk tab.
